Can I be overweight and healthy? | Dr. Bizzy Examines


the idea that someone could be both
overweight and healthy has been around for some time a simple look into workout
culture would show an innate fascination for becoming bigger stronger and fitter
but does packing on the extra weight actually cause more harm than good
latest science is quite clear and detailed the negative effects that
weight gain has on the human body while there is no universal number when it
comes to an ideal weight people should not ignore significant weight gain and
the implications it could have on future health it is incredibly easy to gain weight in
today’s world we kind of live in a food carnival where we are bombarded by
various marketing tools on a daily basis food advertisements are designed to
encourage impulsive consumption they grab your attention always using the
same few colors to keep you attentive and to stimulate your hunger on average
one in three adults eat fast food in any given day and they are not the only
problem childhood obesity is increasing at an
alarming rate major food chain to know this but continue using slide tactics
such as a playful mascot so-called Happy Meals and playgrounds and restaurants to
attract children but more and more kids are entering adulthood 20 pounds heavier
than in the 60s and many have lost the ability to self-regulate food intake by
the time they are 5 supermarkets can be blamed as well they pack their aisles
with foods that are high in sugar saturated fats and sodium because they
taste better and generate more sales many of the major food chains that we
know are operated by corporate businesses whose structure follows a
for-profit model they are aware of the negative effects the food industry is
creating in our society but their main focus is making money not our health and
now we are dealing with a fat epidemic where more and more people are becoming
overweight and obese this not only brings all the physical problems that
are typically associated with obesity or many mental and social consequences as
well people are growing up with less self esteem self worth and self respect
which in turn is increasing the risk for mental disorders such as depression
anxiety and growing up less socially adept the research is there most studies
that discuss weight and weight gain will always mention the negatives associated
with it such as an increased risk for heart disease diabetes stroke and high blood
pressure however in almost all of these studies
the primary tool to measure a person’s weight relative to their height is the
BMI scale also known as a body mass index
it works by measuring your height and weight and then producing a personalised
number along the scale between underweight all the way to obese
clinicians love using this scale as it gives them a basic tool to quickly
assess their patients relative health generally if you are overweight or obese
according to the BMI there is a greater risk for many of the diseases and health
problems mentioned previously but being underweight also has its own risks a
person who is under weight according to the BMI is at greater risk for
osteoporosis eating disorders and nutrient
deficiencies but if you look at the history of some of the best athletes in
the world you might want to question the usefulness of BMI many athletes had a
BMI which would classify them as either overweight or in many cases obese this
brings to debate some inherent problems with BMI for one BMI cannot distinguish
between fat and muscle weight on average muscle weighs about 2.5 times more than
fat and has a whole host of health benefits that works in the opposite
direction and actually causes more harm to the body athletes like LeBron James
and Muhammad Ali who are overweight according to their BMI also are
extremely healthy because they have more muscle and very low body fat a second
drawback to the BMI is that it cannot determine where body fat is located
studies show that excess fat around the abdomen is associated with a greater
risk than fat around the hips and thighs this excess fat around the abdominal
cavity usually refers to visceral fat which is the fat that covers many
internal organs such as the pancreas intestines and liver the more visceral
fat that covers your organs the greater your risk is for increased blood
pressure higher glucose and higher cholesterol levels typically a waist
size greater than 40 inches tends to signify excess visceral fat and
measuring both waist size and BMI can predict greater risk for disease and
death than BMI alone an interesting side note about sumo wrestlers actually
explains this better even though most sumo wrestlers are classified as obese
according to their BMI they do not suffer from many of the afflictions
normally associated with obesity their plasma glucose triglyceride cholesterol
all appear normal even though they may have
miss bellies much of their fat is actually stored right under the skin
also known as a subcutaneous layer and does not actually line many of the
visceral anga areas only when they retire from the sport do we start to see
the health complications associated with obesity as more visceral fat develops
from lack of exercise so so far we’re seeing BMI alone is not really good at
determining a person’s help people who are overweight and obese according to
BMI still look healthy but only if you are a professional athlete or a gym rat
well what about regular ordinary people who don’t really go to the gym in that
case BMI might actually be better to determine if you are overweight and
unhealthy that’s why doctors really like using PMI because more often than not
they are not treating a sumo wrestler or any other professional athlete what are
the odds the next LeBron James comes walking through a doctor’s clinic pretty
slim so doctors will tend to use BMI because
it is a general scale that quickly assesses overall health but just to be
safe doctors should do more comprehensive
tests like ease to get an overall picture of a person’s health another
idea that doctors heavily debate is the concept of metabolically healthy obesity
mhm oh for short that is even though people may be obese according to their
BMI they show no health complications typically associated with obesity the
word metabolic coming from the word metabolism is your body’s ability to
conduct the necessary chemical reactions that keep you alive if you have a high
metabolism your body could convert a lot of the food you eat into energy that is
used to fuel these reactions if you have a low metabolism on the other hand your
body works slower and is left with excess calories coming from the food
that you eat that gets converted into fat over time so going back to
metabolically healthy obesity these people are obese but show normal blood
test results normal vital signs and are generally healthy overall doctors are
extremely careful to label someone as MHO because more often than not people
may only be MHO for a short period of time MHO is described as a transient
state meaning it’s usually only temporary
there tends to be some so of external circumstance that allows
people to be MHO for some time one of which may be exercised as we saw with
sumo wrestlers and many professional athletes these people tend to be obese
based on their BMI but remain relatively healthy throughout their careers only
when they have retired from their sports do we see a greater risk for developing
health complications I like to usually refer to this as the classic big belly
syndrome that we see when athletes retire from their sports when you
compare the fat distribution between a metabolically healthy person and someone
who is metabolically at risk both being obese you can see healthier individuals
tend to have more subcutaneous fat the fat underneath the skin as well as less
visceral fat less liver fat and less muscle fat subcutaneous fat is less
dangerous than visceral and gut fats several physicians have developed a
criteria that tends to fit a profile of someone who has a high BMI and appears
to be metabolically healthy these include people who typically have good
physical fitness a waist size of no more than 40 inches for men and 35 inches for
women normal blood pressure normal total cholesterol level and a normal blood
sugar level as well having a normal sensitivity for insulin a lot of this
can be checked by visiting your doctor and asking for a simple blood test or by
doing a physical examination obese and at-risk patients really don’t fit any of
this criteria one of the keys being is that their insulin doesn’t work as well
and instead of being sensitive enough to detect and regulate blood glucose levels
they have insulin resistance which allows the blood glucose levels to rise
uncontrollably and signals the onset of diabetes this criteria is very useful
for physicians as it allows them to really dictate the direction for
treatment for their patients if a patient is metabolically at risk and
obese the implication is that their lifestyle choices are not really helping
them exercise and diet are not working well enough so physicians may have to
finally resort to gastric bypass surgery as a last-ditch effort to help the
person which ends up being very costly very invasive and pulls his various
risks but if there are patients that can be
identified as metabolically healthy and obese the clinical implication is that
they do not have to immediately resort to gastric bypass but instead Double
Down or intensify the patient’s own lifestyle choices more exercise and diet
changes could be even more beneficial for this person even though BMI and MHO
show that overweight and healthy people do exist do not make the mistake in
assuming this may apply to you the only real way to know for sure is by speaking
to a health care professional here are a few questions to ask next time you see
our doctor at home your scale and clothing are easy ways to measure weight
gain we’ll also look at your everyday movements can you walk up a couple of
flight of stairs or go for a two minute jog around your block without getting
fatigued if not then you should consult your doctor about your weight they can
assess your situation to help you start off with simple dietary changes and
recommend proper exercise programs that will go a long way with your weight
management do not obsess about a small amount of weight gain because it does
happen to everyone just don’t ignore it soon enough
those five pounds can turn to 10 20 or even 50 pounds and sadly in some cases
and early retirement the key is to be mindful and check with your doctor that
you are metabolically healthy and on track to maintaining an ideal weight
